Synapse are able to accept referrals from almost all health funds, Medicare Chronic Disease management plans, Veterans’ Affairs and general visits.
Chronic disease management plans can be made separately for:
- Chronic conditions
- Type 2 Diabetes (group and individual sessions available)
- Aboriginal and Torres Strait Islanders
Exercise physiologists assist chronic conditions such as musculoskeletal conditions, heart disease, arthritis, osteoporosis, asthma, cancer, depression and type 2 diabetes.
DVA cover exercise physiology services to enable you to do a safe exercise program that you can do on your own. This exercise prescription is personalised to your specific condition and situation and may be changed as your condition improves.
All exercise referrals are 1 hr duration.
We consult at the home, workplace and health club and pool. We travel to you because we find this works best. We find a convenient time and place that suits your needs.
Our treatment generally consists of three broad areas:
- Clinical exercise prescription
- Education on health condition and capacity and physical activity
- Behaviour modification
If it is your first consultation with an exercise physiologist, you may be wondering what to expect? We begin consultations with a discussion on your health status and where you would like to be to determine the most appropriate steps.
The first consultation will involve some movement for assessment and potentially your prescription. You do not need any specific clothing (something that allows you to move and feel comfortable) or any specific equipment.
