Synapse are able to accept referrals from almost all health funds, Medicare Chronic Disease management plans, Veterans Affairs and general visits.
All exercise referrals are 1 hr duration.
We travel to your patient. We work with clients who have mobility issues, injuries and older adults and find this works best. We find a convenient time and consult at the home, workplace and gym visits. We also do hydrotherapy sessions where clinically relevant.
Our treatment generally consists of three broad areas:
- Clinical exercise prescription
- Education on health condition, capacity and physical activity
- Behaviour modification
GPs and health professionals refer to us as the benefits of activity are well known yet poorly adhered to. We specialise in activity prescription for clinical populations and focus on quality of life for the patient through increasing health awareness. The goal of our consultations is to improve self-management by finding the correct active treatment. We communicate with GPs on clinically relevant areas to assist in the prevention of other health complications and write concise health reports after inital consultation and consultation program.
